The basic elements that are required for a correctly constructed send off/incident report are:
- Time of the offence
- Location on the field
- Identify the offender
- Description of the offence as per the Laws of the Game
- Provide the correct offence category, (R) code
- Description of the dismissal procedure
- Provide details regarding the opponent if there was serious offence and or injury
- Method of restart
- Score at the time of the incident
- Sign and date report
Below are two examples of Red Card Reports incorporating the key details listed above. They provide an example of how to structure a report no matter the incident. Note the details required in the second example for a second caution, Send Off.
The incident which came under my notice was as follows: In the [Insert the minute of the game. If the send off is in the second half then remember to add the time of the first half. E.g. A send off in the 20th minute of the second half is the 65thminute of a 90minute game] of the game, the [Insert team name] were attacking the [Insert opposing team’s name] goal with play just approximately 5yds outside the [Insert team] goal area. As the [Insert team name] player was moving toward the goal area the [Insert team name of the offender] player, [Insert player’s name, shirt number] grabbed hold of the attacking player’s shirt and pulled him back off the ball, he was the last defender.I stopped play and showed [Insert player’s name and shirt number] a Red Card for Denying an Obvious Goal Scoring Opportunity [Insert (R) code]. The player did/didn’t leave the field without further incident. [Give details if there was any incident as the player left]. [If the offence was a serious challenge and the opponent was injured, insert if the player was able to continue/left the field for treatment/was unable to continue and was substituted or interchanged] Play was restarted with a [Method of restart]The score at the time of the incident was [Insert the score at the time of the incident. Home team score first] Sign and date the report
The incident which came under my notice was as follows: In the [Insert the minute of the game] of the game, I showed [Insert player’s name and shirt number] a Red Card and sent them from the field as I had issued them a second Caution for the match. The first Caution had occurred in the [Insert the minute of the game]. The player did/didn’t leave the field without further incident. [Give details if there was any incident as the player left]. [If the offence was a serious challenge and the opponent was injured, insert if the player was able to continue/left the field for treatment/was unable to continue and was substituted or interchanged] Play was restarted with a [Method of restart]The score at the time of the incident was [Insert the score at the time of the incident. Home team score first] Sign and date the report.
